The following attachments include the current IEMA Initial Damage Assessment forms and accompanying instructions that are to be used by local jurisdictions to document both Individual Assistance (IA) and Public Assistance (PA) damage information following a local disaster. Please disregard all previous forms, as these are the only forms that IEMA will accept.

There are two options for accessing the IA and PA forms. If a local jurisdiction has the Excel '03 version or newer, both forms can be downloaded and completed using that software. For those jurisdictions that do not have Excel version '03 or newer, a link is available at the bottom of this page to download a free copy of the Excel document viewer. This will allow opening and printing the forms, which would be completed manually. Using this option does not allow the forms to be completed using the software. Following completion of these forms, they would be forwarded through the county EMA office to their respective IEMA regional office.
