  About Form IL-1363

Senior citizens and persons with disabilities can use Form IL-1363 to apply for:

  • a license plate discount, and
  • a Ride Free Transit Card.

What are the benefits available?

License plate discount
If your Form IL-1363 is approved, the Illinois Secretary of State’s office will allow you a license plate fee discount for one vehicle.

Ride Free Transit Cards
Individuals are eligible for free rides on all fixed-route public transit systems if they are under age 65, have a qualifying disability and meet the Circuit Breaker income eligibility requirements; OR you are 65 years of age or older and meet the Circuit Breaker program income eligibility requirements.

How and when do I apply for benefits? [ top ]

If you send us your completed Form IL-1363, you have automatically applied for a license plate discount.

To receive a Ride Free Transit Card, you should also complete Section I: Line 35 and 36 (for your spouse).

Your 2011 Form IL-1363 must be postmarked on or before December 31, 2012.
