Forms

Residential, Commercial, Industrial, and Farm appeal forms are available to the public in a variety of ways.

The appeal forms may be requested from the Illinois Property Tax Appeal Board via electronic mail or by calling or writing either the Springfield or Des Plaines office. Click on the Office Locations tab (left) for the addresses and telephone numbers. In your request, please be sure to include your full name, address, day and evening telephone numbers, and the type and quantity of forms you are requesting.

The appeal forms are also available in the Adobe Acrobat Portable Document Format (.pdf). If you would like to download these forms and print them out yourself you may need to download the Adobe Acrobat Reader.

If you have Microsoft Word loaded on your computer, you can download a version of the appeal forms which will allow you to fill them out right on your computer.

It will still be necessary for you to mail your appeal form or hand deliver it. Forms which are telefaxed or e-mailed will not be accepted. The original signature of the appellant or attorney is always required on the appeal form.

If you do not have Microsoft Word, you must use the Adobe Acrobat Portable Document Format version of the form; otherwise, the appeal form will be distorted, and the PTAB will be unable to docket your appeal.
